Care Navigator

Job Summary

A company is looking for a Care Navigator, Temporary.

Key Responsibilities

• Provide direct care navigation services to patients to help them navigate the healthcare system
• Conduct assessments to identify factors impacting patients' health and well-being
• Advocate for patients in accessing community resources and maintain an updated database of these resources

Required Qualifications

• At least 3+ years of experience in care navigation, social work, or community outreach in a healthcare setting
• Start-up experience is a plus
• Fluency in Spanish is preferred but not required
• Experience in an on-demand service or healthcare environment
• Strong organizational skills and ability to thrive in a fast-paced environment
