Job Summary

A company is looking for a Care Navigator for a 3-6 month contract.

Key Responsibilities

• Deliver comprehensive care navigation and access support for members
• Collaborate with cross-functional teams to provide seamless care navigation services
• Educate members on their care referral options and guide high-risk members through their healthcare journey

Required Qualifications

• 2+ years of work experience, preferably in a fast-growth company environment
• 1+ years of experience in healthcare (e.g., social work, managed care, insurance)
• Prior Care Navigation experience strongly preferred
• Ability to operate effectively in a remote environment
• Mission-driven with a commitment to improving the healthcare system
