Job Summary

A company is looking for a Care Navigator for a 100% remote, full-time position.

Key Responsibilities

• Support current patients by answering questions and triaging them to appropriate resources
• Document patient interactions and assist with administrative tasks
• Conduct targeted patient outreach and provide feedback on processes

Required Qualifications

• 3+ years of experience in a healthcare setting, particularly in care navigation and patient support
• Knowledge of healthcare terminology, especially in addiction medicine
• Passion for serving individuals with addiction and mental health challenges
• Experience with technology and common business tools
• Ability to multitask and work independently in a fast-paced environment
