Job Summary

A company is looking for a Care Navigator (Remote).

Key Responsibilities

• Conduct telephonic outreach to members and providers for service coordination and documentation
• Coordinate services and manage scheduling with providers, community partners, and caregivers
• Support member and provider requests, ensuring timely responses and issue resolution

Required Qualifications

• High School diploma or GED required
• One (1) year of related experience in a health-related service field required
• No specific licensure or certification required
• Proficiency in Microsoft Office Suite
• Ability to work independently and manage time effectively
