Job Summary

A company is looking for a Per Diem Care Navigator.

Key Responsibilities

• Process referrals to secondary care and maintain directories of specialists and community services
• Manage communication and documentation for referrals to ensure continuity of care
• Assist in training stakeholders on the external referral process and handle prior authorizations as needed

Required Qualifications

• At least 3 years of applicable experience, preferably in a Primary Care setting
• High school diploma/GED required
• Proficient in Google spreadsheets and/or Excel, database management, and data tracking
