Care Navigator

Job Summary

Supporting patient care coordination in a flexible, part-time capacity, the Care Navigator will manage referrals, appointment scheduling, and follow-up tasks while learning to navigate internal systems and communicate effectively with patients and healthcare teams.

Key Responsibilities

• Manage a moderate caseload of patients requiring coordinated referrals, scheduling, and follow-up
• Accurately document patient interactions and updates in CRMs
• Communicate status updates to patients and internal partners as directed

Required Qualifications

• High School Diploma or General Education Degree (GED) required; college education preferred
• Spanish speaking preferred
