Care Navigator

Job Summary

Supporting Curana providers and care managers with non-clinical tasks, the full-time Care Navigator will manage patient inquiries, coordinate electronic health records, and facilitate communication within the healthcare team in a 100% remote environment.

Key responsibilities

• Address patient and DPOA inquiries and assist with scheduling follow-up appointments
• Manage electronic health records and coordinate necessary documents for provider review
• Collaborate with the interdisciplinary care team to ensure effective communication across the health network

Required qualifications

• High school diploma or equivalent
• 1+ years of experience in a medical office or related field
• 1+ years of experience with Electronic Health Record (EHR) documentation
• Extensive understanding of medical terminology
• Proficient computer skills and ability to adapt to various technology platforms
