Job Summary
Supporting Curana providers and care managers with non-clinical tasks, the full-time Care Navigator will manage patient inquiries, coordinate electronic health records, and facilitate communication within the health network while working remotely.
Key responsibilities
- Address patient and DPOA inquiries via telephone and assist with scheduling follow-up appointments
- Manage electronic health records and ensure accurate patient documentation
- Collaborate with the interdisciplinary care team to support seamless communication across the health network
Required qualifications
- High school diploma or equivalent
- 1+ years of experience in a medical office or related field
- 1+ years of experience with Electronic Health Record (EHR) documentation
- Extensive understanding of medical terminology
- Proficient computer skills and ability to adapt to various technology platforms
