Job Summary
Seeking a full-time Care Navigator to coordinate care for patients and their families remotely, this role involves managing communication, scheduling, and supporting patients through their healthcare journey while documenting care needs and collaborating with clinical teams.
Key responsibilities
- Answer inbound communications from patients, caregivers, and external providers while scheduling and maintaining contact with patients
- Document patient safety, care needs, and caregiver issues under clinical supervision, and report findings to the clinical team for care plan implementation
- Provide emotional support and educational resources to patients and families, ensuring privacy and compliance with HIPAA regulations
Required qualifications
- Prior experience in care coordination or navigation for patients and caregivers
- Ability to communicate effectively in both English and Spanish is a plus
- Experience working with patients facing cognitive challenges
- Strong time management and organizational skills
- Familiarity with electronic health records (EHR) systems
