Care Navigator

Job Summary

To support patient care, the full-time remote Care Navigator will contact recently discharged patients to assist in scheduling follow-up appointments and will coordinate with physician offices to secure these appointments.

Key responsibilities

• Conduct outbound and inbound calls to patients and provider offices
• Utilize computer systems to read charts, fax records, and document patient scheduling preferences
• Support team workload while independently completing daily assignments

Required qualifications

• Minimum 18 years old with a high school diploma or equivalent
• Preferred completion of at least 1 year of college
• Ability to troubleshoot basic computer issues
• Comfortable navigating multiple computer applications
• Self-motivated and responsible with a people-first mentality
