Care Navigator

Job Summary

To support patient care post-hospitalization, the full-time Care Navigator will conduct telephonic outreach to members, guiding them on utilizing plan benefits and ensuring timely follow-up with healthcare providers, all while working remotely within the U.S.

Key responsibilities:

• Guiding members on utilization of plan benefits such as follow-up with PCP, transportation, and medication follow-up
• Completing the FMC assessment in Pathway and accurately documenting member conversations
• Transferring members to member services when needed for follow-up

Required qualifications:

• High School Diploma / GED OR equivalent work experience
• Must be 18 years of age OR older
• 2+ years experience in a medical office, Skilled Nursing Facility, Assisted Living Facility, or outpatient clinic
• 2+ years of experience handling inbound and/or outbound calls with patients/members
• Ability to work an 8-hour schedule during the work hours of 7:30am-5:00pm Pacific, Monday-Friday
