Care Navigator

Job Summary

To support a growing healthcare practice, the part-time hourly Care Navigator will assist with patient referral coordination, appointment scheduling, and follow-up tasks while learning to navigate internal systems and workflows remotely.

Key Responsibilities

• Manage a moderate caseload of patients requiring coordinated referrals, scheduling, and follow-up
• Document patient interactions and updates accurately in CRMs
• Communicate status updates to patients and internal partners as directed

Required Qualifications

• High School Diploma or General Education Degree (GED) required; college education preferred
• Spanish speaking preferred
