Care Navigator

Job Summary

To support a growing healthcare practice, the part-time Care Navigator will engage patients, coordinate their care, and connect them to necessary resources while working remotely and potentially visiting a wellness center in Victorville, CA.

Key responsibilities

• Conduct proactive outreach to schedule patient visits and ensure timely preventative care
• Coordinate care management with Nurse Practitioners, handling patient follow-ups and scheduling
• Maintain accurate documentation of patient interactions and adhere to HIPAA standards

Required qualifications

• 1-2 years of post-high school education or an associate's degree; bachelor's degree in a related field is a plus
• 1-3 years of experience in a patient-facing healthcare role
• Familiarity with medical terminology and chronic disease management in older adults
• Outstanding phone skills with sensitivity toward protected health information (PHI) and HIPAA compliance
• Must be organized, detail-oriented, and able to multitask in a fast-paced environment
