Care Navigator

Job Summary

Managing the referral process for patients needing care outside of Crossover's clinics, the full-time Care Navigator will coordinate specialist appointments, maintain directories, and ensure effective communication between patients and healthcare providers while working remotely.

Key responsibilities

• Process daily referrals to secondary care and maintain high-quality directories of specialists and community services
• Communicate with patients and specialists to ensure timely appointments and continuity of care
• Assist in training stakeholders on the external referral process and troubleshoot any complications that arise

Required qualifications

• At least 3 years of applicable experience, preferably in a Primary Care setting
• High school diploma or GED required
• Proficient in Google spreadsheets and/or Excel, database management, and data tracking
