Care Navigator

Job Summary

Managing the referral process for patients needing care outside of Crossover's clinics, the full-time Care Navigator will coordinate with specialists, maintain directories, and ensure timely communication and documentation while working remotely.

Key responsibilities

• Process daily referrals to secondary care and maintain high-quality directories of specialists and community services
• Ensure timely communication and documentation of referrals to facilitate continuity of care
• Train stakeholders on the external referral process and assist with prior authorizations as needed

Required qualifications

• At least 3 years of applicable experience, preferably in a primary care setting
• High school diploma or GED required
• Proficiency in Google spreadsheets and/or Excel, database management, and data tracking
