Care Navigator

Job Summary

To support older adults in their mental health journey, the full-time remote Care Navigator will guide patients and their families through the intake process, ensuring they feel understood and supported while coordinating care and matching them with appropriate therapists.

Key responsibilities

• Guide patients and caregivers through the intake process with empathy and professionalism
• Coordinate care and match patients with the appropriate mental health professionals
• Provide emotional support and reassurance during initial interactions with the organization

Required qualifications

• Strong interest in mental health and healthcare access
• Experience in healthcare, customer service, or administrative support is welcome but not required
• Highly organized and detail-oriented with the ability to manage multiple patients
• Comfortable with technology and learning new software tools (e.g., Google Workspace, EHR platforms, CRMs)
• Empathetic communicator who can handle emotional conversations with kindness
