Job Summary

A company is looking for a Care Navigator I - RN to provide case management support and ensure the best continuum of care for injured workers.

Key Responsibilities:

• Conduct initial reviews of referrals and assess injured worker needs
• Establish and monitor treatment programs, coordinating services and resources
• Collaborate with interdisciplinary teams, communicate progress, and prepare discharge plans

Required Qualifications:

• R.N., L.P.N., or M.A. with a minimum of two (2) years of experience
• Knowledge of company products, services, and business operations
• Excellent customer service and communication skills
• Computer navigation and operation skills
• Ability to analyze information and make decisions based on medical need and customer request
