Job Summary

A company is looking for a Care Navigator Manager to lead their Care Navigation team in a remote setting.

Key Responsibilities
  • Manage and oversee the operations of the Care Navigation team, including hiring, training, and performance management
  • Ensure high-quality patient coordination, including appointment scheduling and follow-ups, while addressing escalated patient concerns
  • Collaborate with clinical leadership to improve processes, optimize workflows, and enhance patient and caregiver satisfaction
Required Qualifications
  • 5+ years of experience in care navigation, patient coordination, social work, or healthcare team management
  • Demonstrated success in people management and team leadership
  • Strong knowledge of dementia, Alzheimer's, or complex/chronic care coordination
  • Bachelor's or Master's degree in a relevant field (e.g., Public Health, Social Work, Healthcare Administration, Nursing, Counseling)
  • Proficiency with EHR systems and task management tools
