Care Navigator (Outbound)

Job Summary

A company is looking for a Care Navigator (Outbound) to support a team of clinicians in delivering complex care plans remotely.

Key Responsibilities

• Conduct telephonic communication with patients and families, including triaging calls and initiating outreach
• Develop and manage patient care plans in collaboration with patients to achieve health goals
• Provide care coordination as a liaison between patients, families, and providers, ensuring effective communication

Required Qualifications

• Associates degree or higher education
• Additional certification as a BSW, CNA, LPN, or MA is highly required
• Prior healthcare experience, ideally with chronically ill patients, is required
• Bilingual English/Spanish is preferred
• Strong customer-service orientation and exceptional organizational skills
