Job Summary

A company is looking for a Care Navigator (Outbound) to provide telephonic and administrative support for patient care coordination.

Key Responsibilities
  • Conduct telephonic communication with patients and families, including triaging calls and initiating outreach
  • Develop and manage patient care plans while collaborating with patients to establish health goals
  • Act as a liaison between patients, families, and providers to ensure effective communication across the care team


Required Qualifications
  • Associates degree or higher education
  • Additional education/certification as a BSW, CNA, LPN, or MA is highly required
  • Prior healthcare experience, ideally with chronically ill patients, is required
  • Bilingual English/Spanish is preferred
  • Strong customer-service orientation and exceptional organizational skills
