Job Summary
To support a team of clinicians in delivering complex care plans, the full-time remote Care Navigator (State Licensed) will provide telephonic and administrative support, coordinate care delivery for patients, and manage patient care referrals and appointments.
Key responsibilities
- Conduct telephonic communication with patients and their families, including triaging calls and initiating outreach
- Develop and manage patient care plans while collaborating with patients to achieve their health goals
- Act as a liaison between patients, families, and providers to ensure effective care coordination and communication
Required qualifications
- Associates degree or higher education
- Additional education/certification as a BSW, CNA, LPN, or MA is highly required
- Prior healthcare experience, ideally with chronically ill patients
- Bilingual English/Spanish is preferred
- Strong organizational skills and ability to multi-task
