Job Summary
A company is looking for a Care Partner to engage members and navigate them through their cancer journey.
Key Responsibilities
- Engage with members to assess their health and social needs, and connect them to appropriate resources
- Handle inbound and outbound calls in a high-volume environment while managing time effectively
- Collaborate with an interdisciplinary team to provide holistic support throughout the member's care journey
Required Qualifications
- At least 2 years of experience in a patient-facing role related to care coordination or healthcare navigation
- Experience in a metrics-driven, fast-paced environment such as a contact center
- Ability to build rapport quickly with patients in a primarily phone-based setting
- Comfort with technology and experience with electronic health records or similar tools
- Previous experience working in a remote role with a quiet and secure working environment
