Care Partner

Job Summary

Engaging with members throughout their cancer journey, the full-time Care Partner will navigate healthcare resources, assess needs, and provide support via phone, text, and email in a remote environment.

Key responsibilities

• Conduct outreach to members to identify health and social needs, supporting advanced care planning and resource connection
• Serve as the primary point of contact for members, collaborating with an interdisciplinary team to ensure holistic support
• Manage a high volume of inbound and outbound calls, documenting interactions and coordinating care effectively

Required qualifications

• At least 2 years of experience in a patient-facing role such as care coordination or community health work
• Proven success in a metrics-driven, feedback-oriented environment
• Experience in assessing and addressing both expressed and unexpressed needs of patients/members
• Ability to build rapport quickly in a high-volume, primarily phone-based setting
• Familiarity with technology and comfort learning new tools, including electronic health records and communication platforms
