Care Services Manager

Job Summary

A company is looking for a Manager, Care Services.

Key Responsibilities

• Conduct specialized care consultations and needs assessments for individuals and families affected by ALS
• Manage a portfolio of clients and ensure their needs are met through appropriate programs and services
• Coordinate referrals and partnerships with healthcare providers and community agencies

Required Qualifications

• Bachelor's Degree or related experience in social work, case management, health administration, or a related field
• Minimum of 4 years of relevant professional experience or equivalent education and experience
• Experience with progressive neurodegenerative diseases is preferred
• Knowledge of Medicare, Medicaid, and patient assessment skills is preferred
• Knowledge of ALS is a plus, but not required
