Job Summary

A company is looking for a Care Support Specialist to assist in delivering coordinated services to clients and families.

Key Responsibilities

• Support the Care Support Partner in delivering coordinated services to clients and families
• Serve as an initial point of contact, responding to inquiries with empathy and professionalism
• Collect and input vital client information into the secure client portal and assist with documentation management

Required Qualifications

• Bachelor's degree or equivalent experience in Human Services, Healthcare Support, Customer Service, or related fields
• 2 years of experience in a client-facing or service support role, preferably in a healthcare, nonprofit, or social services setting
• Demonstrated commitment to empathy, confidentiality, and professionalism
• Ability to work independently while contributing to a collaborative team
