Job Summary

Supporting members' social and care management needs, the full-time remote Care Team Operations Associate will coordinate home health services, manage referrals, and connect with insurance providers to ensure members receive appropriate medical services.

Key responsibilities

• Coordinates home health, insurance, vendors, and other social assistance services for members
• Initiates and manages referrals based on member needs, ensuring effective communication throughout the process
• Uses analytical platforms to generate and communicate weekly reporting to support clinical care

Required qualifications

• 1 year of administrative work experience, including scheduling and coordination tasks
• Experience in maintaining accurate records and documentation
• Familiarity with health insurance and social assistance services
• Ability to communicate effectively with members and external agencies
• Proficiency in using analytical and member benefits platforms
