Care Wellness Education and Coaching Specialist

Job Summary

A company is looking for a Care & Wellness Education and Coaching Specialist.

Key Responsibilities:

• Deliver person-centered care management services to long-term care insurance policyholders
• Provide topic-based education, resources, and suggestions for successful aging in place to policyholders and caregivers
• Collaborate with policyholders and their families to establish goals and action steps

Required Qualifications:

• At least 2 years of supervised care management experience with older adults and their families
• 5-7 years of professional experience working with older adults and their families in various care settings
• College degree in a field related to care management or equivalent work experience
• Strong knowledge of care, treatment, community programs, and equipment used to assist individuals with functional and cognitive impairments
• Strong knowledge of senior/geriatric/home health care, long-term care insurance, and/or Medicare
