CareConnect Coordinator

Job Summary

To support the urgent care program, the part-time CareConnect Coordinator will manage dispatch communications, triage requests, and schedule in-home visits for CareConnect RNs across a multi-state region in a fully remote capacity.

Key responsibilities

• Act as the central hub for urgent care dispatch requests, prioritizing and managing multiple simultaneous requests efficiently
• Utilize geographic knowledge and mapping tools to optimize travel routes for clinicians and ensure timely dispatching
• Maintain effective communication with Urgent Care Clinicians and clinical leadership while navigating multiple software platforms

Required qualifications

• High school diploma or GED required; Bachelor's degree preferred
• 2+ years of experience in a healthcare practice or hospital setting preferred
• Prior experience in medical dispatch, logistics, or care coordination strongly preferred
• Strong geographic understanding and spatial reasoning skills
• Proficiency in Microsoft 365, Electronic Health Records, and scheduling software
