Job Summary

A company is looking for a Case Management Analyst (Remote).

Key Responsibilities

• Conduct annual Health Risk Surveys to identify member needs
• Coordinate care activities and inform care managers of health/safety risks
• Engage members and document care management processes in compliance with guidelines

Required Qualifications

• 2+ years in behavioral health, social services, or a related field
• Proficient in Microsoft Office Suite
• Access to a private, dedicated workspace
• High School Diploma with equivalent experience required
• Associate's or Bachelor's Degree in behavioral health or human services preferred
