Job Summary
A company is looking for a Case Management Coordinator.
Key Responsibilities
- Conduct comprehensive evaluations of referred members' needs and recommend case resolution approaches
- Coordinate and implement care plan activities while monitoring progress
- Consult with multidisciplinary teams to overcome barriers and escalate quality of care issues as needed
Required Qualifications
- Bachelor's degree in behavioral health or human services, or a non-licensed master-level clinician
- 2 years of experience in behavioral health, social services, or a related field
- Confidence in working independently and using virtual collaboration tools
- Proficiency with MS Word, Excel, Outlook, and PowerPoint
- Experience in case management and discharge planning
