Case Management Specialist

Job Summary

A company is looking for an Analyst, Case Management Specialist (Remote, EST).

Key Responsibilities

• Interact with low stratification members via phone, coordinating care and updating assessments and care plans
• Create care plans based on assessments and claims data
• Participate in Interdisciplinary Team meetings to discuss member health care needs and barriers to care

Required Qualifications

• 3+ years of experience in a health-related field
• 2+ years of experience with Microsoft Office applications (Word, Excel, Outlook)
• Bachelor's Degree or equivalent experience required
• Familiarity with community resources and services
• Strong organizational skills and ability to work independently
