Job Summary

A company is looking for a Bilingual Cardiovascular Clinical Guide (Health Coach).

Key Responsibilities

• Screen members for eligibility in the Hypertension Clinic and conduct comprehensive hypertension assessments
• Deliver intensive lifestyle and nutrition counseling and coach members toward realistic health goals
• Coordinate multidisciplinary care and serve as the primary point of contact throughout the care journey

Required Qualifications and Education

• Certified health coach required
• At least 2 years of work experience in longitudinal chronic disease management
• Spanish bilingual
• Basic understanding of hypertension and cardiovascular disease
• Experience caring for Medicare patients
