Certified Care Coordinator

Job Summary

To support patient care management, the full-time Certified Care Coordinator will conduct monthly outreach for a panel of 400-450 patients, providing health coaching and collaborating with clinical teams, all while working remotely from a HIPAA-compliant home office.

Key responsibilities:

• Manage patient outreach and conduct daily outbound calls to ensure continuity of care
• Collaborate with clinical teams to address patient needs and support care coordination goals
• Document all patient interactions in real time while maintaining HIPAA compliance

Required qualifications:

• Must hold a nationally recognized medical assistant certification or Certified Pharmacy Technician license
• Minimum 1 year of clinical experience in a healthcare setting
• Experience with Electronic Medical Records (EMRs) and strong computer navigation skills
• HIPAA-compliant home office setup with minimum internet speed requirements
• Must be legally authorized to work in the United States
