Certified Care Manager (CMA)
Job Summary
Supporting patients through telephonic outreach and care coordination, the full-time Certified Care Manager (CMA) will monitor care plans, document in EHRs, and facilitate access to medical services while working remotely.
Key responsibilities:
- Manage and support patients' healthcare needs via telephonic outreach
- Monitor adherence to care plans and evaluate patient progress in a timely manner
- Educate patients and caregivers about relevant community resources and coordinate continuity of care
Required qualifications:
- CMA Certification is required and must be maintained
- Minimum of 1 year work experience in a healthcare setting with complex chronic disease states preferred
- Chronic Care Management experience is required
- Proficiency with personal computers and experience with Microsoft Excel preferred
- Technical and analytical skill set is necessary
