Certified Clinical Coding Specialist

Job Summary

A company is looking for a Clinical Documentation & Coding Specialist.

Key Responsibilities

• Perform comprehensive chart preparation for dementia-care patients, reviewing clinical histories and documentation
• Identify undocumented chronic conditions and prepare findings for provider review
• Accurately assign ICD-10-CM codes and ensure compliance with CMS HCC guidelines

Required Qualifications

• High school diploma required; Associate's or Bachelor's degree in a health-related field preferred
• Active CPC or CCS certification (AAPC or AHIMA); CRC certification strongly preferred
• 2-3+ years of medical coding experience, including 1-2 years in HCC/risk adjustment
• Strong understanding of ICD-10-CM, HCC models, and CMS/HHS risk adjustment principles
• Experience coding neurology, psychiatry, behavioral health, or dementia conditions is strongly preferred
