Job Summary
To enhance patient care and quality outcomes, the full-time Certified Clinical Quality Coordinator will work remotely, serving as a Subject Matter Expert on quality improvement measures, conducting patient outreach, and managing quality data collection and reporting.
Key responsibilities
- Conduct proactive patient outreach initiatives to educate on preventive care and facilitate adherence to treatment plans
- Collect and manage quality data using various systems, ensuring accuracy and compliance with quality reporting requirements
- Communicate quality performance metrics and improvement strategies with internal teams and external customers
Required qualifications
- Associate degree or higher in a related field, or certification as a licensed professional (CMA, LPN)
- 1-3 years of experience in a healthcare setting with knowledge of medical terminology and quality measures
- At least 2 years of experience in Medicare Advantage quality data abstraction and patient outreach
- Proficiency in managing health data and navigating multiple systems, including EMR and Microsoft 365 applications
- Experience in quality improvement initiatives and data submissions required by health insurance payers is preferred
