Certified Community Health Worker

Job Summary

To support high-quality, patient-centered care, the remote Certified Community Health Worker will engage with patients, coordinate care, and connect them to community resources while collaborating with an interdisciplinary team to address Social Determinants of Health.

Key responsibilities

• Conduct patient engagement and outreach to establish trust and provide support for those with high social determinants of health needs
• Serve as a liaison between patients, families, and clinical teams to facilitate seamless transitions of care and assist with enrollment in health and social services
• Identify and address social, economic, and environmental factors impacting patient health through comprehensive assessments and resource connections

Required qualifications

• 3+ years of experience in healthcare coordination, community health work, or a related field
• Credentialed Community Health Worker certification
• Experience with a team-based approach to care and excellent knowledge of community resources
• Strong communication and teaching skills, with the ability to prioritize and manage multiple tasks
• Intermediate skills with MS Office Suite and the ability to work effectively in a remote environment
