Certified Community Health Worker - PA

Job Summary

Providing outreach to patients with complex medical and social needs, the full-time Certified Community Health Worker - PA will assist in navigating the healthcare system, conduct assessments, and collaborate with multidisciplinary teams while working remotely from Pennsylvania.

Key responsibilities:

• Orients and assesses new Community Health Workers while supporting their training and proficiency
• Identifies and engages medically complex patients through various outreach methods, including home and hospital visits
• Facilitates telemedicine visits and collaborates with the clinical team to ensure adherence to treatment plans

Required qualifications:

• High School Diploma or Equivalent (GED) is required
• Minimum of 1 year of related work experience is required
• Certified Community Health Worker (CCHW) certification from the Pennsylvania Certification Board is required
