Job Summary
Empowering members to manage their diabetes, the full-time remote Certified Diabetes Educator will guide a panel of patients through their healthcare journey, providing education and counseling via virtual communication to help them achieve stability and self-management.
Key responsibilities
- Screen members for Diabetes Clinic eligibility and conduct detailed assessments
- Provide ongoing education on diabetes management, including medications and lifestyle counseling
- Collaborate with healthcare team members to develop care plans and optimize medication regimens
Required qualifications
- Active unrestricted Compact Registered Nurse license or Registered Dietitian license
- Active Certified Diabetes Care and Education Specialist (CDCES) certification
- At least 2 years of experience in clinically managing diabetes
