Certified Diabetes Educator (Spanish)

Job Summary

Providing diabetes education and support remotely, the part-time Certified Diabetes Educator (Spanish) will work with patients to improve adherence and health outcomes while meeting program metrics and quality requirements.

Key responsibilities

• Provide diabetes education support to enhance patient adherence and health outcomes
• Establish rapport and therapeutic alliances with a diverse patient population via phone
• Utilize motivational interviewing techniques to facilitate behavior change among patients

Required qualifications

• Current certification as a Certified Diabetes Care and Education Specialist from the NCBDE
• Fluency in Spanish
• Proven experience using motivational interviewing for positive behavior change
• Ability to work independently and manage time efficiently
• Comfort with advanced technology including Electronic Medical Records and Microsoft Office
