Certified Health Advisor

Job Summary

A company is looking for a Health Advisor to provide care coordination and support for at-risk patients.

Key Responsibilities

• Conduct post-discharge follow-up calls with patients to assess needs and reinforce care plans
• Review and reconcile discharge medications and assist in coordinating follow-up appointments
• Identify high-risk patients and connect them with appropriate care management resources

Required Qualifications

• Certified Emergency Medical Technician (EMT) or Medical Assistant (MA)
• Preferred 3-5 years of experience in an outpatient healthcare setting
• Experience with electronic medical records (EMR) systems
• Proficient in basic technology platforms and able to navigate multiple systems
