Certified Life Care Planner

Job Summary

To support the evaluation of long-term care needs, the remote Certified Life Care Planner will create evidence-based life care plans, review medical records, and prepare detailed reports for various stakeholders.

Key responsibilities

• Review detailed medical records to identify treatment history and care needs
• Prepare life care plans, medical cost analyses, and related research reports
• Evaluate complex medical files to determine current and future care requirements

Required qualifications

• Active Registered Nurse license required
• Certified Life Care Planner certification required
• 10+ years of clinical, case management, rehabilitation, or related healthcare experience
• Strong understanding of medical terminology and long-term care considerations
• Excellent written communication skills for preparing detailed reports and analyses
