Job Summary

A company is looking for a Transitions of Care Associate to support members navigating the healthcare system after hospital discharge.

Key Responsibilities

• Contact patients post-discharge to assess their well-being and resource needs
• Coordinate services and care with affiliated vendors for members
• Assist in scheduling post-discharge visits with primary care and specialist providers

Required Qualifications

• Active CMA/RMA certification from a nationally accredited agency
• 2-3 years of experience in healthcare, managed care, home health, or community health settings
• Fully bilingual in English and Spanish (desired)
