Job Summary

A company is looking for a Medical Assistant - Clinical Navigator.

Key Responsibilities

• Support delivery of healthcare by performing activities under the direction of the Nurse Practitioner
• Provide one-on-one coaching and support to patients managing chronic conditions
• Conduct comprehensive assessments and monitor patients' progress and adherence to treatment plans

Required Qualifications

• Active Certification as a Medical Assistant from an accredited association
• 5+ years of experience as a Medical Assistant
• Highly skilled in behavioral-based coaching
• Possesses excellent clinical acumen and competency in patient assessment
• Experience in patient education for individuals managing chronic conditions
