Job Summary

A company is looking for a Medical Assistant - Clinical Navigator.

Key Responsibilities

• Support healthcare delivery by performing activities and procedures under the direction of Nurse Practitioners
• Provide one-on-one coaching to patients managing chronic conditions and assist in executing personalized care plans
• Conduct assessments and monitor patients' progress, adherence, and health outcomes through regular check-ins

Required Qualifications, Training, and Education

• Active Certification as a Medical Assistant from an accredited association
• 5+ years of experience as a Medical Assistant
• Experience in patient education for individuals managing chronic conditions
• Prior experience working in a high-growth, fast-paced startup environment
• Tech-savvy and capable of working with multiple systems
