Key Responsibilities

• Support healthcare delivery by performing activities under the direction of the Nurse Practitioner
• Provide one-on-one coaching to patients managing chronic conditions and assist with personalized care plans
• Conduct assessments and monitor patient progress through regular check-ins and remote tools

Required Qualifications

• Active Certification as a Medical Assistant from an accredited association
• 5+ years of experience as a Medical Assistant
• Highly skilled in behavioral-based coaching
• Possesses excellent clinical acumen and competency in patient assessment
• Experience in patient education for individuals managing chronic conditions
