Job Summary

A company is looking for a Medical Assistant - Clinical Navigator.

Key Responsibilities

• Support delivery of healthcare by performing activities and procedures under the direction of Nurse Practitioners
• Provide one-on-one coaching and support to patients managing chronic conditions
• Conduct comprehensive assessments and monitor patients' progress through regular check-ins

Required Qualifications

• Active Certification as a Medical Assistant from an accredited association
• 5+ years of experience as a Medical Assistant
• Experience in patient education for individuals managing chronic conditions
• Tech-savvy and capable of working with multiple systems
• Prior experience supporting patients remotely is a plus
