Key Responsibilities

• Support patient enrollment and maintain engagement for chronic care management
• Conduct outreach to patients through text and phone, reinforcing care plans and identifying barriers to adherence
• Document patient interactions and ensure compliance with billing and HIPAA standards

Required Qualifications

• Certified Medical Assistant
• 1-3 years of healthcare experience
• Chronic disease or nephrology experience preferred
• Ability to work in EHR and CCM platforms simultaneously
• Intermediate skills in MS Office Suite
