Key Responsibilities

• Support patient enrollment and eligibility verification for Chronic Care Management (CCM)
• Conduct patient outreach and engagement through text and phone communication
• Document patient interactions and ensure compliance with billing and HIPAA standards

Required Qualifications

• Certified Medical Assistant
• 1-3 years of healthcare experience
• Experience in chronic disease or nephrology preferred
• Ability to manage workflows in EHR and CCM platforms
• Intermediate skills with MS Office Suite
