Key Responsibilities

• Engage patients remotely and in person to navigate health needs and care plans
• Coordinate care by facilitating communication between patients and healthcare providers
• Document interactions and assessments accurately in electronic health records

Required Qualifications

• Active and current Certified or Registered Medical Assistant
• 3+ years of experience as a Medical Assistant in an outpatient setting
• Knowledge of medical terminology and primary care workflows
• Familiarity with motivational interviewing and patient education principles
• Strong customer service orientation
